Welcome to AHE’s Roadmap Implementation Center (RIC), a comprehensive platform that provides practical materials and instructions to put the Roadmap into action in your own organization or a multi-organization collaborative.

Familiarize yourself with the Roadmap to Advance Health Equity before exploring the RIC. Open the interactive Roadmap graphic, then select each component to learn key information that will set you up for success. Once you’re oriented, continue below.

Where to start, and in what order to work

The Roadmap to Advance Health Equity guides organizations—individually or collaboratively—through targeted activities to sustainably reduce or eliminate health inequities. The Roadmap is designed to be worked through sequentially, with each component building on the last. Relatedly, the RIC will make the most impact if its components are followed sequentially, as well.

Begin with Foundational Activities to set goals, define roles, and establish ways of working. You might consider revisiting the Foundational Activities documents as needed throughout your team’s care transformation process.

Second, move to Essential Elements, which has themes that run across every component, Teams should should start early with partnering with communities, earning stakeholder buy-in, and anticipating data needs and opportunities and continue to refer to them throughout their intervention. This upfront investment strengthens the work and supports lasting impact.

Next, go to Create Cultures of Equity. The concepts in this component are essential for all other Roadmap work and should be applied early. From there, teams should Identify a Health Equity Focus and then continue working through the remaining Roadmap components from top to bottom as displayed in the Roadmap graphic.

How to get the most out of the RIC

Try not to skip or rush any Roadmap components. Each one is evidence-based and serves a clear purpose. Skipping steps lowers your chances of meeting your goals. If you need to delay or cannot fully implement a component, make a plan to come back to it. For example, if you can’t partner with patients and communities during your root cause analysis, return to it later after you have recruited patients and community members to update and strengthen your analysis and reprioritize your root causes based on their feedback. Then you can update and improve your care transformation and payment changes based on what you learn.

The resources throughout the RIC were developed for the AHE Learning Collaborative. The term “members” is used to refer to patients. The term “collaborative” is used to refer to the implementing team or organization.

Additional Guidance and Support

How long will it take to work through RIC materials?

Health equity efforts vary based on a team’s focus, capacity, structure, and external factors. The timelines in the Roadmap are written as general guidance, not a madate. Many teams revisit components as they learn more, refine their approach, and respond to new challenges. Continual iteration is expected and supports more thoughtful, sustainable equity work over time.

What are some tips for staying organized?

Teams should save all of their work, including their team charter, in a centralized drive or folder. That way, all team members can access the material whenever necessary to review, revise, and update key documents. As teams evolve, future members can access the materials to learn about the initiative and how it operates.

How can my team gauge progress and success?

Use the Goal and Objective Setting tool to track progress across Roadmap components. Teams set high-level goals in their Team Charter, then create clear process goals for each component to support those larger aims. Together, these goals help teams monitor progress, adjust their approach, and stay focused on meaningful outcomes.
