Our vision is a world where all communities and people who have been the most impacted by multiple systems of oppression have access to the resources and care they need to thrive and live with abundant health. AHE exists to inspire innovation and collaboration to advance health equity nationwide and promote a healthcare system without inequities. Our mission is to transform patient care by bringing together multi-partner state teams of state Medicaid agencies, health plans, healthcare delivery organizations, patients, and community-based organizations. We help them develop, align, and implement payment reform and care transformation that address the medical and social needs of patients and communities, within an anti-racist culture of equity.

We meet health care organizations where they are at in their health equity journeys; we work them within their sociopolitical contexts and guide them where they need to go.

  • The specifics of how AHE’s Learning Collaborative (LC) teams designed their care transformations using their root cause analyses

Each webinar will touch on various aspects of AHE’s Roadmap to Advance Health Equity and will feature members from AHE’s LC team who will discuss their team’s specific health care transformation as well as challenges they’ve encountered and wins they’ve realized along the way. Attendees will leave each interactive webinar session with practical ideas and solutions that they can apply to their own efforts to advance health equity.


Advancing Health Equity through Patient and Community Partnership

This webinar will highlight learnings from the Advancing Health Equity Program, including tools and resources that organizations can access for their own efforts related to care delivery and payment transformation. Representatives from state teams will share specific examples of their community partnership efforts related to health equity intervention design, focusing on concrete steps they have taken to partner with the communities they serve. Attendees will learn more about:

  • connecting with populations with lived experience and strategies for including them in the planning and implementation phases of care transformation. 
  • pitfalls to avoid when designing and implementing partnership efforts.
  • the positive impact meaningful partnerships with patients and community members has on health and health care transformations.

Transforming Care to Advance Health Equity

  • key components for conducting an effective root cause analysis with an equity lens to diagnose existing health and healthcare inequities to inform care transformation activities.
  • how state-based teams specifically designed their care transformations using their root cause analysis.

Payment Transformation & Contracting Approaches to Advance Health Equity

This 60-minute webinar will explore how to design and implement payment reform mechanisms that support and incentivize the transformation of healthcare delivery to reduce health inequities by meeting medical and social needs, highlighting the work of AHE’s Learning Collaborative teams.
