Supported by the Robert Wood Johnson Foundation through Finding Answers:Disparities Research for Change, the authorship team for this supplement provides a cutting-edge, systematic summary of intervention strategies to promote equitable health outcomes through evidence-based care for communities of color. Focusing on the selected conditions of cardiovascular disease, diabetes, depression, and breast cancer, this work serves as a tangible reminder that although progress has been made, much remains to be done. In addition, we need similar reports for persons with health conditions other than those covered in this supplement, including older patients with complex comorbidities.
